INFORMED CONSENT:

Your desire for a reversal of permanent sterilization represents a change in you heart. It is important that your expectations match my ability. Not every procedure is a success. Even when you develop sperm in the post operative ejaculate, pregnancy does not always occur - one factor that we all minimize since we are quick to to things in our own strength is - "God opens and closes the womb." You and I are not in control in spite of what we think!

The complications that I have experienced in vas reconstruction are as follows:
1. Swelling - 100%
2. Pain - 90%
3. Hemorrhage - to some degree in every case, but those requiring drainage and control are less than 1%
4. Testicular atrophy - rare
5. Loss of testicle - 2 in 2000 cases
6. Infection - about 1%, and those have responded to antibiotics
7. Death - I have read about one case in the world literature. You have to evaluate this against the background of a million vasectomies being done each year in the U.S. I did not share this to frighten you, but to adequately inform you of all potential hazards.

I understand the above complications and risks and give Cary L. Leverett, M.D. permission to do a vasectomy reversal upon me.
